Exploring association of Inter Lukein-28 B Polymorphism with HCV treatment outcome in case of direct acting antiviral therapy.

Abstract


Background: Association of genetic polymorphism with HCV treatment outcome has been explored in many of the studies, using drugs like Interferon (IFN) and Ribavirin but least studies have been done to explore its association with direct acting antiviral (DAA) drugs that is Sofosbuvir(SOF) and Ribavirin in combination. Aims and objectives: This study was aimed to explore the association of IL-28B polymorphism roles as predictor of response in case of DAA therapy. Methodology: This study was composed of 182 patients having chronic HCV infection and 59 control patients who were free of active HCV infection. Polymorphism of IL-28 B at rs12979860 and rs8099917 SNPs was determined in 101 and 81 patients respectivelyFor polymorphism genomic DNA was extracted and multiplex PCR was done using Amplisense, HCV FRT IL-28 and MYgo Real Time PCR system. Results: Among the cytokines genotype, CC frequency was higher among the control groups (49%) compared to CHC patients (24%), having significant association (0.001), while CT was similar in both CHC (30.3%) and control groups (30.5%) and TT was comparatively again higher in CHC (45.5%) as compared to control group (20.3%), . In case of rs rs8099917 SNP, genotype frequencies among CHC and healthy groups were as, TT was more in control group, while TG and GG were more in CHC patients, showing significant association (0.003). Among total of 101 CHC patients, 83 (82%) patients showed SVR while 18 (12%) patients did not achieve SVR. Il-28 B SNPs among the SVR and relapsed patients were as, among total of 24 CC genotypes, 23 (96%) CC genotypes were associated with SVR while only one (4%) was with relapse. Similarly, CT and TT genotype frequencies among SVR and Relapse patients were 78%, 71% and 22%, 29%, 28.2% respectively. All these results are showing greater association (<0.05). Conclusion: These results showed that CC genotype was mostly associated with HCV clearance and also with SVR while TT was associated with severity of disease and relapsed conditions
